Child Development Check-In

The Child Development Check-In is an opportunity for child development consultation, for children ages 3 and up, prior to entering Kindergarten. This is a process to check in regarding a child’s development and to ensure access to resources available to support each child’s continued growth.

If you are a family member, please contact your child's EC4T site.

If child has an Individualized Education Program (IEP), please contact your child’s IEP manager.

If child does NOT attend EC4T and/or Head Start and does NOT yet have an Individualized Education Program (IEP) please complete the following form:
